My Physio My Health – Terms and Conditions

1. Acceptance of Terms
By using the services provided by My Physio My Health, you agree to be bound by the following Terms and Conditions. These terms may be updated from time to time, and it is your responsibility to review them periodically.
2. Services
My Physio My Health provides physiotherapy services at 12 clinic locations in Adelaide, Australia. Services may include physiotherapy assessments, treatments, and other related healthcare services including remedial massage, exercise physiology and dietetic consults.
3. Appointments
Appointments can be scheduled by contacting us at (08) 7093 0405 or through our online booking system. Clients are encouraged to arrive on time for appointments. In case of emergencies, please contact us as soon as possible.
For clients covered by the National Disability Insurance Scheme (NDIS) or Medicare’s Chronic Disease Management Plan, My Physio My Health welcomes appointments under these schemes. Please inform us of your NDIS or Medicare EPC status when scheduling appointments so that we can provide the necessary support and coordinate with the respective processes.
4. Payment and Fees
Payment for services is due at the time of the appointment. Fees for services may vary, and information about fees will be provided upon request. My Physio My Health accepts various payment methods including credit cards, EFTPOS, and cash. My Physio My Health is equipped with HICAPS facilities at all clinic locations, allowing eligible clients to process health insurance claims on-site.
5. Cancellations and Rescheduling
Clients are required to provide at least 24 hours’ notice for appointment cancellations or rescheduling. Failure to do so may result in a cancellation fee.
6. Privacy and Confidentiality
My Physio My Health respects your privacy and maintains the confidentiality of your personal information in accordance with applicable privacy laws. For more details, please refer to our Privacy Policy.
7. Code of Conduct
Clients are expected to behave in a respectful and courteous manner during appointments. My Physio My Health reserves the right to refuse or terminate services if a client’s behavior is deemed inappropriate. Harassment of our staff or other clients will not be tolerated.
8. Intellectual Property
All content and materials provided by My Physio My Health, including but not limited to logos, text, video, and images, are the property of My Physio My Health and are protected by intellectual property laws.
9. Limitation of Liability
My Physio My Health is not liable for any direct or indirect damages arising from the use of our services. Clients use our services at their own risk.
10. Governing Law
These Terms and Conditions are governed by the laws of South Australia, and any disputes will be subject to the exclusive jurisdiction of the courts of South Australia.
